APPLICATION PROCESS
1.
The Foundation utilizes a formal application process to
administer it's grant program. The application form is only
available from the Foundation office. Non-profit
organizations that meet all of the qualifications described in
the Grant Guidelines are invited to submit a request for a
grant application. Requests for an application are accepted
between May 15 – July 15 for consideration during the Fall
grant cycle, and November 15 – January 15 for consideration
during the Spring grant cycle. Requests can be sent to the
Foundation’s e-mail address or office address. The request
should include the name of the organization, contact name
and title, mailing address, telephone number, and e-mail
address. Organizations contacting the Foundation
electronically should clearly display the name of the
organization in the subject line. The request should include
a brief description of the organization’s mission and how
funding from the prospective grant would be applied. The
length should not exceed one page.

The Foundation offers two grant cycles during the year with
grants awarded in June and December.

Our Grant Application is available as a pre-formatted MS
Word 2003 document and will be sent electronically unless
the organization does not have Internet access and
specifically requests the application to be sent through first
class mail delivery. The application is not available for
download from this website.

An organization that wishes to be considered for a grant
award must submit a complete proposal using our
application form. No substitutions please. Proposals must
be received by the assigned due date. Proposals received
after the due date will be considered late and not accepted.

Grant writers should note the specifications for length and
font size in Sections 4 and 5 of the Grant Application. Due
to the high volume of proposals received during each grant
cycle we regret that we are unable to consider additional
pages other than the supplemental attachments required in
Section 8. Note: The Foundation does not return any
material included with the proposal unless specifically
requested, and only at the organization’s expense.

Grant proposals are only accepted in hard copy through
U.S. Mail or overnight mail delivery unless prior
arrangements are made to personally deliver the proposal
to the Foundation office. Unfortunately, we cannot accept
any proposal received by electronic means (e-mail or fax).
Any proposal sent electronically will be declined. The
proposal, nor any of its contents should not be bound or
stapled. Single copy only please.
7.
A confirmation notice will be sent shortly after receipt of the
proposal to each submitting organization that provides an
e-mail address. We may alternately choose to contact your
organization by telephone if additional information or
clarification is required in processing the proposal.
8.
The Foundation may contact an organization at any time
during the review process to conduct an interview or
arrange a site visit. The purpose of the interview or site
visit is intended solely as a means for gathering additional
information and should not be construed as an indication
the grant request will be approved.
9.
Due to the high volume of proposals a competitive grant
application process is used. As a result the Foundation is
not able to support all the many worthwhile projects and
organizations that submit requests.
10.
The Foundation does not assume an obligation for
permanent support of any activity, and prefers not to make
multi-year grants.
11.
The Foundation will notify each organization that
submitted a proposal during the grant cycle by telephone
or written correspondence to advise the outcome of the
board’s decision. Notice will occur by approximately the
second week of June for the Spring grant cycle, or the
second week of December for the Fall grant cycle.
12.
Organizations successful in receiving a grant from the
Foundation are required to acknowledge receipt of the
check on letterhead stationery.
13.
An evaluation report is due at the earlier of: (1) completion
of the grant project; (2) submission of a subsequent grant
proposal; or (3) at 12-month intervals until all funds have
been expended. The Grant Evaluation Report Form
will be sent with the grant award notification.
